Forms

DCM offers considerable expertise, equipment and specialized space to support your in-vivo projects. For easy access to our most commonly requested services, all of our requisitions are made available on Blackboard or contact us for support.

Animal Import/ Export Request

You must have a valid Animal Use Protocol to import animals. Initiate the process by submitting an order requisition (see LARS DCM Investigator User Guide, section 2.2.1) on LARS. DCM’s Animal Coordinator will contact you once the requisition is place. The Coordinator will ensure all health documentation is provided by the shipping institution and then reviewed by a DCM Veterinarian. Note that your import may be subject to other requirements such as material transfer agreements that can lengthen the overall process for importation. Animals from non-commercial sources will be subject to quarantine.

If a colleague at another institution has made a request for your mice, please submit an external transfer request (see LARS DCM Investigator User Guide, section 2.2.2) on LARS. DCM’s animal coordinator will be in touch with next steps on your import/export requests once your requisition form has been received.

Before animals are acquired or used, all procedures must be subject to ethical review in accordance with federal guidelines (Canadian Council on Animal Care) and provincial legislation (Animals for Research Act- OMAFRA) on animal care and use. No procedures can be undertaken without prior review. Ethical review ensures that all animal use meets federal guidelines, provincial statutes, University of Toronto guidelines, and best practices reflected in standard operating procedures.

The deadline for Canadian suppliers is 11:00 am Thursday for delivery the following week. Facility Access

If you require access to the CCBR or MSB animal facility, you will need a fob. Fobs are obtained through your departmental business officer. Fobs must not be shared. Once you have your fob, access to the facilities may be provided by requisition. Please contact DCM to obtain a requisition form. Office hours for this service may be obtained as indicated on the requisition, Tuesday and Thursday 1-3pm.

Note that training is a prerequisite to access to facilities. All animal users must also be included on valid Animal Use Protocols.

Access to the MSB and CCBR animal facilities is restricted in order to assure consistent control of environmental variables and to minimize factors that may alter experimental results. Investigative personnel are permitted access to the facility in which their animals are housed after they have completed training. DCM management is unable to authorize access without prior training.

Internal Animal Transfer

To transfer animals to another AUP or to another PI, complete an internal transfer request (see LARS DCM Investigator Guide, Section 2.2.2) through LARS. Both parties must have a valid Animal Use Protocol to transfer animals. Please complete the requisition 2 days prior to transfer date. Submit the form to the DCM receiving clerk and mark cages for transfer. In addition, animal numbers must be appropriately debited and credited in the relevant protocols in My Research (MR). Contact the Research Oversight and Compliance Office for assistance in reconciling the animal numbers.

New Account Request Internal/ External

Please provide 48hrs notice for all requisitions. This ensures that all required space, equipment and supplies are available for your use.

Training Registration

All investigative personnel using animals must be adequately trained prior to beginning their work in keeping with federal guidelines on animal care and use. The DCM training program is modular. At a minimum you must complete the lecture module and one biomethodology module.  Additional training is required for more advanced techniques, for use of chemical or biological hazards, or any procedures not covered in the introductory biomethodology module. All modules include an evaluation to establish competency.
